At Homewatch CareGivers
we pride ourselves in the rigorous process we use to ensure that whatever
home care plan you choose, that it is specifically tailored to your
individual needs and wishes and those of the family. That way no two
packages are exactly the same.
Our step by step process consists of:
Step 1 Initial enquiry and
assessment: Following the initial enquiry, one of
our coordinators arranges an appointment for our nurse at the client’s
own home to assess your overall needs. From the information gathered
through listening and understanding your requirements our nurse will
prepare a specific Care Plan which will then be explained in as much
detail as is required in the presence of other family members if required.
Step 2 Consultation and evaluation:
This consultation visit is free and arranged at a suitable time for
both the client and any family member. It allows us to clearly understand
your specific needs and helps us develop a specific tailored Care
Plan for you. In addition, we can carry out a home safety and fall
risk evaluation identifying any significant concerns that we may have
and that should be remedied. This report will be provided to you.
Step 3 Written personalised
Care Plan
After this meeting a written proposal follows within 24 hours. This
allows both the client and family members to clearly see and understand
what is being proposed and identify all of the associated costs. Also,
additional services that are available are highlighted so that if
at a later stage any further services are required they can be simply
chosen as their need arises. This ensures that the Care Plan is dynamic
and has the ability to be flexible as the circumstances and needs
of the client inevitably change over time.
Step 4 Care Service Agreement
Only after the client and other family members involved fully understand
the schedule, services and costs is this document signed.
Step 5 Matching and selection
of caregiver
Once the agreement is signed we select from our pool of caregivers
the most suitable person based on personality, skills, expertise,
availability, and interests.
Step 6 Caregiver briefing and
orientation
The selected caregiver is fully briefed on the client by the coordinator
in advance. Then they are introduced to the client and confirmation
is given of what will be involved in the delivery of the clients care.
In addition, our report forms are explained so that these can be monitored
by the client, members of their family and our coordinators.
Step 7 Quality assurance
Scheduled and unscheduled visits are made to the clients’ homes
to evaluate the assistance being given, to consider any changing needs,
and the overall relationship between the client and the caregiver.
